TRAVELLING ALLOWANCE :<\/strong>
\nFor SO\/C (Medical-Gen. Medicine), TO\/C & Station Officer\/A : The Outstation Candidates called for Interview will be eligible for to and fro rail fare for IInd Class or actual fare by the shortest route whichever is less, subject to the production of tickets \/ticket\u00a0numbers etc. For Post Nos. 03\u201304 and 06-18 : Only outstation SC\/ST Candidates called for Written\u00a0Examination\/Interview will be eligible for to and fro rail fare for IInd Class or actual fare (GENERAL CLASS) by the shortest route whichever is less, subject to the production of tickets\/ticket\u00a0numbers etc.However, Traveling Allowance is not admissible to those SC\/ST candidates who are already in Central\/State Government services, Central\/State Government Corporation, Public Undertakings, Local Government Institutions and Panchayats.<\/p>\n

HOW TO APPLY\u00a0:<\/strong><\/p>\n

Application\u00a0should be made on good quality white A-4 size plain paper and should be in conformity with the format prescribed in proforma given overleaf. Candidates are advised to ensure that the\u00a0application\u00a0is legible, preferably typed in English or Hindi only.<\/p>\n

The Candidate\u2019s Name, Address with Pin Code, Date of Birth should be written legibly in English in bold capital letters, even if a Candidate fills-in\u00a0application form\u00a0in Hindi.<\/p>\n

The\u00a0Application\u00a0and the outer cover should be super scribed as \u201cPost No. ________________ Name of the Post________________ against Advertisement No. HWB\/01\/2012\u201d<\/strong>.<\/p>\n

Only one\u00a0application\u00a0should be submitted for one post. Separate\u00a0application\u00a0should be submitted for each post if a candidate wishes\u00a0to apply\u00a0for more than one post.<\/p>\n

PHOTOGRAPH:<\/strong>\u00a0A recent passport size photograph should be affixed on the right hand top corner of the\u00a0application. One additional copy of passport size photograph with name of the candidate written on its back side, should also be sent along with the\u00a0application.<\/p>\n

ATTESTED COPIES OF CERTIFICATES OF :<\/strong>
\ni) Educational \/ Technical qualification (supported by appropriate mark sheets indicating the subjects\u00a0offered at the examinations).
\nii) Date of birth proof.
\niii) Experience Certificates specifically stating the kind of work done should accompany the\u00a0application.

\nv) Certificates issued by the Competent Authority regarding the claim that the candidate belongs to SC, ST or OBC [i) Non-Creamy Layer ii) Validity certificate should have been issued within one year]in the prescribed proforma.
\nvi) Certificate from Appropriate Authority regarding physical disability (applicable to physically disabled persons only).
\nvii) Discharge from Defence Service (applicable to ex-defence personnel only).
\nviii)Proof if affected by 1984 Riots.
\nix) Certificate regarding domiciled in Kashmir Division from 01.01.1980 to 31.12.1989 (if claiming age relaxation for Kashmir Division).<\/p>\n
